The Silent Shift

Within the quiet cage of bone and breath,
A silent shift can pave the path to death.
No longer just a “fatty” state of old,
Steatotic Liver Disease unfolds—
The broader mantle where the shadows grow,
As cellular accumulations show.

When metabolic fires begin to fade,
And insulin’s strict orders are delayed,
MASLD arrives with weight and rising blood,
Drowning the working cells in fatty flood.
Yet if the glass is emptied day by day,
ALD will wear the strength away;
And where the two in toxic union blend,
The hybrid MetALD marks a troubling trend.

When simple storage turns to fierce ignite,
MASH brings the swelling and the cellular fight.
Inflammation sparks a hidden flame,
And fibrosis becomes the scarring’s name.

Then comes the late stage, hardened and austere:
Cirrhosis binds the tissue year by year.
At first, compensated, it hides the strain,
Performing silent duties through the pain.
But soon, decompensated, boundaries break—
The failing organs tremble in its wake.
With ascites filling, veins that bleed and tear,
Encephalopathy’s fog and jaundice in the air.

At End-Stage Failure, functions wholly cease,
And only a transplant can offer peace.
Yet hope remains before the final key:
In early steps lies reversibility.
